Update 4’s headline: a medical scene and a new ambulance on a lag-tuned city.
Update 4 is the first Harbor East patch with a public, specific RP toy: a hospital scene and an ambulance, plus lag work so that building can actually fill. This page is how to use those toys. Rank the roles on emergency and pay context on jobs.
What Update 4 actually shipped
From the live experience description (still current in August 2026):
- New RP scene: Hospital
- New ambulance
- Optimized lag for smoother gameplay
It did not ship a documented promo code, a voice-chat radio, or a listed surgical minigame. If your group needs a diagnosis loop, invent it in character — that is the point of a scene, not a missing wiki row.
How to run a shift
- Call it in chat. “Hospital open” on a 25-player server will pull two or three people. Silence means you are doing homework in an empty wing.
- Staff the door. One person on reception, one on the ambulance. More than four staff and nobody is left to be a patient.
- Park the ambulance like a prop with physics. Do not store it inside Style Central. Drive to the plaza or Tool Shop, pick up a downed player, return.
- Treat, then discharge. Harbor East lists mild blood and repeated mild violence. Keep scenes PG-violent; the maturity rating is not a gore sim.
- Clear the bay so the next pair can play. Average session length city-wide is about 15 minutes — plan a short shift.
Ambulance etiquette
The ambulance is new, so everyone will steal it once. House rules that work:
- If you are not on EMS, do not spawn-camp the garage.
- Sirens in the plaza are a scene. Sirens through someone’s mansion tour are grief.
- You cannot voice-call the driver. Text “en route” because VC is unsupported.
- Lag was a stated problem; if the van jitters, wait a second before you jump out. That is why the patch mentioned optimization.
Pairing hospital with other lanes
Emergency only pops when civilian or criminal scenes create patients. A yacht pizza party (see the official waterfront screenshot) will not fill a trauma bay. A Tool Shop fight will. Hosts should pick one “main stage” per hour — hospital or hood or mall — using the session planner.

Running the first medical night on a new account
You do not need VIP or a mansion to staff Update 4. Spawn, walk until you see civic architecture or an ambulance, and post in chat. If nobody answers, be the patient for two minutes so a stranger can try the van — that single loop teaches the building better than a map marker we do not have.
Bring a simple outfit. Giant accessories clip in doorways and make ragdolls worse if lag spikes. The official patch mentioned optimization because this interior is new; you can still help by not parking five sports cars in the bay.
If your group wants diagnoses, agree on three fake conditions before you start (sprain, faint, “car vs palm”). Keep them mild to match the rating. Then actually drive. The ambulance is the feature; standing in a lobby named Hospital is not.
When the shift ends, park the van where the next server can find it. A missing ambulance is the fastest way for the next 25 players to assume Update 4 “does not work.”
What we will not invent
No unverified list of “Code Blue rooms,” organ minigames, or NPC disease tables. If Century Operators adds those, they will land in Update 4 notes and this page together. Until then, props + players + ambulance = the feature.
Gear and pay
Bring whatever RP tools the phone gives you. Do not buy Ultra Money Speed expecting a hospital multiplier; the pass is global earnings, not a medic trait. VIP’s luxury house is a nice break room, not a clinic.
